With modern scientific research proving the safety of silicone implants, most women choose silicone breast implants over saline. However, each implant type has benefits and disadvantages. One benefit of saline is its safety in case of a rupture. The body will simply absorb the saltwater. The disadvantage of saline implants is that they do not feel as natural as silicone breast implants. The benefit of silicone breast implants is their softness. If ruptured, it is much more difficult to remove the ruptured silicone material. With extremely small breasts, silicone implants will feel much softer since all one can essentially feel is the breast implant. In patients who have moderately sized breasts before breast augmentation, the natural breast tissue camouflages the breast implant to a greater capacity than in those with pre-operatively small breasts. Therefore, saline and silicone breast implants will both feel soft and natural in patients who start with large B cup breasts before breast implant surgery. The size of the incision for saline and silicon breast implants is different. Saline implants require the smallest of incisions. Silicon breast implants come pre-filled and require a 3 to 4 cm long incision.

Physicians who wish to move to a pure Private Pay practice and still take Medicare patients must privately contract with such patients. If still accepting Medicare, it is advisable to follow the Medicare fee schedule as to avoid causing any red flags to be raised. The rules related to treating Medicaid patients on a Private Pay basis will vary from state to state. It is advisable to contact the state Medicaid agency for their terms on charging patients directly.
